Aetna Medicare has partnered with specific vendors to offer healthy home visits on its behalf — in the comfort of your home. This program doesn’t replace your relationship with your primary care physician (PCP). Instead, it helps Aetna work with your PCP to manage your care and direct you to the health programs and services you need.
The benefit is offered at no extra cost to you
Participation in this program does not impact your coverage
Our partner Signify Health will contact you. First, they’ll send you an introductory letter followed up with a phone call to schedule your visit
The licensed clinical professional will visit you in the comfort of your home at the scheduled time
After the visit, we’ll mail you a summary of your visit. We’ll also send a copy of the summary to your PCP
Receive helpful coronavirus (COVID-19) education and best practices to stay safe
Ask any health care questions you have
Review your medicines and dosages
Talk about getting the health resources you need
Discuss how to set up a safe, healthy home
Because your health is always our top priority, we take many steps to make sure you feel safe at home. Our licensed clinicians follow all Centers for Disease Control and Prevention (CDC) guidelines and safety measures including, but not limited to, social distancing with anyone else in the household and wearing face masks and gloves.
Your plan may also include telehealth virtual provider visits.
