Zostavax and Shingrix are both covered under Medicare Part D benefits at preferred brand cost share. We encourage you to get Part D vaccines at network pharmacies for cost savings and convenience (you won’t need to submit a paper claim form).

Get your vaccine at a network pharmacy and pay just your cost share (covers both the vaccine and injection).

Visit a network pharmacy for vaccine and go to your doctor for injection

Get the vaccine through a network pharmacy and have your doctor perform the injection. You pay the pharmacy cost share for the vaccine (preferred brand) and the doctor administration fee for the injection. You can then request reimbursement for your doctor’s fee by submitting a Direct Member Reimbursement form. You’ll be reimbursed for the allowable amount that Aetna pays the provider; any amount above the allowable amount is your responsibility.

Get the vaccine and injection from your doctor. You pay for both the vaccine and injection. You can then submit a Direct Member Reimbursement form. If you paid out of pocket for the vaccine, you’ll be reimbursed up to the amount of your cost share. For the injection, you’ll be reimbursed for the allowable amount that Aetna pays the provider; any amount above the allowable amount is your responsibility.
