This team has nurses, medical directors, pharmacists and social workers – all committed to understanding your unique circumstances. They work closely with your doctor and help you achieve your personal best health.
From chronic conditions – like arthritis, diabetes or hypertension (high blood pressure) – to a more serious diagnosis, like cancer, our clinical team will be there for you. They’ll find tools to help you be self-sufficient. And they'll give you relevant information to help you make informed decisions about your health. If it’s your first diagnosis for a condition, our clinical team can work closely with your doctor as you take a new path in your health journey.
We know health care can be complex. That’s why in more critical cases, we also involve a personal nurse case manager.
As your advocate, your case manager:
- Coordinates your care
- Gives you information about treatment options
- Helps you find access to services
- Keeps you informed about costs
- Puts together a plan to help you succeed at your health goals
